Should a postmortem identify one person as the root cause?
Focus on the technical and organizational conditions that allowed the failure. Accountability for follow-up does not require an unsupported blame claim.
Why do postmortem actions fail to prevent recurrence?
They may be vague or unowned. Define the change, responsible role and evidence that it addresses the observed mechanism.
