Should every role appear in every RACI cell?
No. Assign roles according to the actual decision and workflow. Unnecessary consultation can obscure who is responsible for moving work forward.
Why does the matrix still leave approval unclear?
Identify the actual accountable decision role and resolve competing assignments. A list of several approvers may need an explicit approval process.
